Maintenance of Class III Trifurcated Molars Versus Implant Placement in Regenerated Extraction Sockets: Long-Term Results of 2 Cases

2011

Studies to date have reached differing conclusions regarding the long-term prognosis of teeth with class III furcation involvement. Replacement of such teeth with implants could be an alternative. This report compares the treatment outcomes of 2 cases with similar disease progression: 1 treated by implant therapy and 1 maintained with nonsurgical periodontal treatment. Two patients with advanced chronic periodontitis and class III furcation involvement of all molars were treated. Case 1 received a conservative periodontal and antibiotic treatment, followed by 15 years of maintenance. In case 2, the molars were extracted and replaced with implants, and the implants were observed for 7 years.…

Mandibular Molar Root Resection Versus Implant Therapy: A Retrospective Nonrandomized Study

2009

AbstractSuccess rates for both periodontal and implant therapy are often dependent on site and tooth type. For periodontally involved mandibular molars, the decision to hemisect or to extract and place an implant is often complicated. The purpose of the present study was to evaluate the outcomes of the aforementioned treatment modalities for mandibular molars in a private practice setting. A retrospective chart review was performed. In one group of patients (n = 32), 56 mandibular first or first and second molars were treated by hemisection (Group H). A second group (n = 28) received 36 implants in the mandible to replace periodontally involved first or first and second molars (Group I). Al…

Periodontal disease affecting tooth furcations. A review of the treatments available

2007

The molars are the teeth that suffer the greatest periodontal destruction in untreated patients. When periodontal disease affects the furcation of a tooth, the chance that it will be lost increases considerably. An increase in the exposed root surface, anatomical peculiarities and irregularities of the furcation surface all favor the growth of bacteria. These problems make it harder for the patient to maintain hygiene, and impede adequate treatment. The treatment of furcations affected by periodontal disease is one of the most difficult problems for the general dentist and periodontist. The motivation of both the attending professional and of the patient are therefore of great importance. N…

Evaluation of a porous hydroxyapatite alloplast in the management of grade II furcation defects in molars: a case series

2012

Introduction: The present case series evaluates the efficacy of a hydroxyapatite graft material in the management of grade II furcation involvement in first molars. Material and methods: Eight patients presenting with a total of 9 grade II furcation defects in relation to the facial aspect of either maxillary or mandibular first molars were treated with a porous hydroxyapatite alloplast. The clinical outcomes were measured in terms of change in probing pocket depth and clinical attachment level (vertical and horizontal) at 6 months post-operatively. Radiographs were used as supplements. Results: At 6 months, there was a mean pocket depth reduction of 3.12±1.25 mm, a mean vertical attachment…

Predictors of long-term outcomes in patients undergoing periodontal maintenance

2017

Abstract Aim This retrospective study aimed to characterize the baseline status of patients following periodontal maintenance, analysing the association between the long‐term outcome of these patients, smoking, bruxism, and the main clinical and radiographic variables. Material and methods A sample of 174 patients with moderate to severe periodontitis was refined into homogeneous subsamples according to smoking and bruxism and the rate of tooth loss due to periodontal disease (TLPD): 0, 1–2, and >2 teeth. The association and the distribution (χ² test) of the variables within the subsamples were analysed. Results Smoking and bruxism were significantly associated with higher TLPD rates. Verti…
